A reliable and valid tool in capturing the symptoms and signs of peripheral neuropathy is the Toronto Clinical Neuropathy Score (TCNS). This score will provide you with an accurate picture of the percentage of sensory loss in your patient. But only if you perform this exam correctly. 

This score will then be your baseline and instrumental in determining the length of care required to restore lost sensation and/or diminish the associated pain from nerve atrophy.
